- For Individual TPA: TPA ID Card
- If Corporate TPA: TPA ID Card and Employee ID Card
- Patient photo identity proof: PAN Card
- Address proof for patient
- All your previous medical records: Doctor consultation papers, relevant investigation reports, Doctor's referral letter (if any)
- List of the medications currently being taken by patient
- Birth certificate and photo (for child below age 5 years)
